Revisional Options After Laparoscopic Sleeve Gastrectomy (LSG) Failure
If laparoscopic sleeve gastrectomy (LSG) does not achieve the desired weight loss, patients may opt for revisional procedures like RNY Bypass, Mini Bypass, Duodenal Switch, or a re-sleeve. A thorough evaluation of the patient’s medical history can often reveal if the current sleeve is ineffective. These three options can help boost weight loss:
- Gastric Sleeve to Gastric Bypass – Transitioning from a sleeve to either a gastric bypass or mini bypass has proven to be one of the most effective revision options.
- Gastric Sleeve to Duodenal Switch – This procedure, known as biliopancreatic diversion with a duodenal switch, offers a comprehensive approach for substantial weight loss.
- Re-Sleeve – Ideal for patients whose original sleeve has stretched or was not performed correctly. The re-sleeve can be done laparoscopically and, in some cases, through endoscopic techniques (mini-ESG).
Reasons for Vertical Sleeve Gastrectomy (VSG) Failure may include:
- Incorrectly sized sleeve
- Stomach stretching and hormonal adaptation
- The body adjusting to reduced calorie intake
If a Roux-en-Y gastric bypass does not result in adequate weight loss or leads to excessive weight loss, a revision to another procedure may be necessary. While gastric bypass is considered semi-permanent, patients can transition to different options, including the duodenal switch or placing a Lap-Band around the stomach to support further weight loss. Additional revision techniques include:
- Stoma Shrinkage via Sclerotherapy – This involves injecting a sclerosant to reduce the size of the stoma.
- Laparoscopic Pouch Reduction – A surgical method to shrink the stomach pouch.
- Endoscopic Gastric Outlet Reduction (TORe) – Reduces the size of the gastric outlet endoscopically.
- Lap-Band Placement – A band can be placed around the stomach for added restriction.
- Roux Limb Lengthening – Extending the Roux limb to enhance malabsorption and induce weight loss.
Gastric Bypass Failure can be due to:
- Enlargement of the stomach pouch
- Disruption of the staple line
- Stoma enlargement or malfunction
- The body adapting to lower calorie intake
The growing consensus among patients and surgeons is dissatisfaction with gastric banding as a long-term obesity treatment. Many patients experience either inadequate weight loss or complications. The most common revision surgery for Lap-Band patients is transitioning to a gastric sleeve, which maintains the familiar restriction of the Lap-Band but adds the benefit of appetite suppression. Options for revisional surgeries include:
- Re-adjusting Lap-Band Placement
- Converting Lap-Band to Gastric Sleeve
- Converting Lap-Band to Gastric Bypass
Statistics show that within 10 years post-operation, about 60% of patients have their Lap-Band removed. Reasons for Lap-Band Failure include:
- Band Erosion
- Band Slippage
- Pouch Enlargement
- Difficulty in accessing adjustments
- Lack of significant weight loss in certain patients
Vertical Banded Gastroplasty doesn’t produce significant weight loss and can allow patients to regain their weight.
Surgeries VBG can be revised to:
- Gastric Sleeve Surgery (Likely)
- Gastric Bypass Surgery (Less Likely)
