SASI-S Surgery in Mexico
Single Anastomosis Stomach-Ileal Bypass (SASI-S) is a modern bariatric procedure that combines the benefits of a gastric sleeve and an intestinal bypass to the lower small bowel (ileum). It helps you eat less by physically limiting stomach capacity and induces early satiety, while reducing calories and fat absorption.
This procedure yields significant weight loss and high remission rates for diabetes, often with fewer nutrient deficiencies than traditional bypasses. It is performed laparoscopically and is often reversible. Mexico Bariatric Center at Hospital Azar® is the pioneer in SASI-S Surgery in Mexico.
How It Works
SASI-S is a hybrid weight-loss surgery designed for obese and super-obese patients.
The procedure involves two main steps:
- Sleeve Gastrectomy: Approximately 70% of the stomach is removed to create a smaller, sleeve-shaped pouch (restrictive component).
- Intestinal Bypass: The lower part of the stomach is connected directly to the ileum (small intestine), allowing some food to bypass a portion of the digestive tract (malabsorptive component).

Key Benefits
SASI-S in Tijuana, Mexico delivers exceptional clinical outcomes, with patients achieving an average of 87% excess weight loss at one year and 91% at two years, alongside a remarkable 90% remission rate for Type 2 Diabetes and other metabolic comorbidities. Anatomically, because a portion of ingested food continues to travel through the natural duodenal pathway, patients face a significantly lower risk of malnutrition and enjoy better absorption of essential nutrients like iron compared to traditional bypasses.
Furthermore, connecting the sleeve directly to the small bowel relieves pressure along the staple line, substantially reducing the likelihood of leaks and chronic acid reflux, while still remaining a flexible option that can be safely reversed or revised if necessary.
